Starting HRT: What the First Month Actually Looked Like

Handwritten police case file log listing dates, case numbers, offenses, investigators, locations, statuses, and notes
A detailed handwritten log of police cases from 1961 to 1962

The Patch Log is where I document what I’m doing, not what you should do. Start there.

I started hormone therapy this spring after a conversation with my provider that I’d been putting off for longer than I should have. I’m on a low-dose estradiol patch and progesterone. Nothing dramatic. The entry point, more or less.

Here’s what month one looked like, filed honestly:

Week one. Nothing noticeable. I don’t know what I was expecting — some kind of immediate signal — but the body doesn’t operate on that schedule. Applied the patch as directed. Moved on.

Week two. Sleep improved slightly. Not dramatically. Enough to notice. The 2am wake-up happened once instead of four times. I don’t know if that’s the patch or the fact that I’d also reduced caffeine that week. The variables are a problem.

Week three. The brain felt quieter. Less static. I wrote more in five days than I had in the previous two weeks. That could also be deadline pressure. See: variables.

Week four. A bad week. Tired, irritable, one significant sleep disruption. Went back through the log and found that I’d also had a high-stress work week, eaten badly, and skipped two days of movement. The patch is not a correction for not taking care of yourself. That was a useful data point.

One month in: cautiously interested. Not converted. Continuing to track.

This is a personal log of one person’s experience. HRT decisions belong between you and your provider. The Patch Log exists because I wanted to read something like it and couldn’t find it.
